Steps for enrollment:

1. Call the center so we can talk to the caregiver. This phone call will help us determine if the person is appropriate for our services.
2. Setup an appointment with or without the prospective participant. At this visit we will give the caregiver the application and review the requirements and explain the forms. We are required to do what the state calls is a prescreening form.
3. If the prospective participant did not come to the first appointment then a second appointment is required. Setup an appointment so we can physically see and meet the prospective participant. This helps us see that they will stay in the building and their behaviors are appropriate for a group environment. We also want the person feel comfortable here. We also observes things the caregiver often over looks because they are in the middle of the care.
4. Once this is all done the caregiver needs to return the completed application and finish all the required paperwork for the enrollment and setup a start date.

A physician does not need to refer a person to the program. There are not referrals required to seek services with us.

Enrollment is a process.
